"As treated" analysis, fixed cohort analysis, and NNH?

I had a few questions in regards to this study: http://www.bmj.com/content/346/bmj.f880.pdf%2Bhtml

They conducted an "as treated" analysis and also a fixed cohort analysis. I have done a lot of digging and all the information I can find on the two topics don't seem to correlate to this study design. If anyone can help explain why the authors chose these types of analyses that would be great.

I also had a question about the NNH they calculated. They stated it was 1700 need to be treated for one additional hospitalization due to AKI. No matter what variables I try and plug into the equation I cant figure out how they came up with the number 1700. If anyone knows which variables they used to get the number 1700 that would be great!

Thanks in advance for any help!
