Well, could you perhaps be a bit more specific? If you have run
a Cox regression, then I suppose you are somewhat familiar with
the basic ideas of survival analysis/Cox regression models, so
where's the actual problem?
A one unit increase in creatinine (whatever that may be) is associated with a (0.996 - 1)*100% = -0.4% decrease in the hazard of <whatever your event is>.
